I am not getting any response from the addon.
When I open it, the menu pops up but if I click on Movies or TV Shows I get the dialog asking me if I am sure I want to update the ratings, I click yes and then absolutely nothing happens,

You're using an older version of the script (1.2.4), please download the most recent version (1.2.9) from the first post!

OK. So used to getting auto updates through repos. I simply forgot to update, All works well except top 250 won't run. Seems I'm not the first with this issue though
EDIT - I spoke to soon. Now it runs but it always tells me 0 movies were updated and for TV shows it just repeatedly asks me the names of the show until I give up.

Checks the path of php-cgi.exe (in Windows the whitespace in the path are problematic)
Launches the script with the debug mode on and post a new log.

Moved PHP from "C:\Program Files\" to "C:\" and changed the settings in Ratings Updater. Same issue.
Enabled debug logging under "Setting-System-Debugging"

I've investigated and the script works well.
The problem is the php-cgi.exe execution, your command line is "C:/PHP/php-cgi.exe" -q C:\Users\DK\AppData\Roaming\XBMC\addons\script.ratingupdate\resources\lib\php\top250.php but it fails.
You can try this... run php-cgi.exe from the terminal or double click it in an open window, if the result is an error for a missing msvcr110.dll, you need VS C++ 2012. Go here: http://www.microsoft.com/en-us/download/details.aspx?id=30679 and download the x86 version if you install the 64bit version it will give the same error.
I hope that the problem will gone away!

I would have never thought I was missing the redistributable, especially since this script used to run with no issue.
Top 250 and movie ratings are both working now, but it still doesn't seem to recognize any of my TV shows. This is the least of my issues, but figuring it out might help us both.

It's very strange! In linux works well but in Windows if I launch the command from terminal works, in XBMC the same command don't wants to work (with the debug mode ON, the data field is empty, but if you run the command - that you find in xbmc log - from terminal the data are taken!).

Next week! I'm very busy with my job, this week-end I'll try to work on the addon! In the meantime you can run the top250 and movies, leaves tv shows untouched for now!

the script could have promblems updating the mpaa ratings and it's descriptions lately ?
I just noticed it but i cant fugure out where the problem is.
