Couple of things to check, I presume you have set a static ip address for the mythtv-backend.
1. Mythbackend
Using mythtv-setup
Check that you have set the ip address(192.168.1.99) in two places on the first screen in General, the first ip at top of screen is selectable, the 2nd at bottom of the screen has to be typed in.
Check that the Security Pin is set to 0000
2. Mysql
Use mythbuntu-control-centre MySQL section and make sure it is set to enable (MySQL on ethernet interfaces..) for the Master Backend Role
Now reboot the machine and try kodi again.

Thanks, Item 2 solved my problem. That step did not seem to be documented in the tutorials I was referencing.
