It is all or nothing. So if you run an Export then three minutes later clear the watched status of a few movies, then you need to run the export again to update the nfo files.

I did have a functioning update, but even with the most current watchdog (it's an outside set of code) the problem reported there persisted. It appears to be limited to that one OS. I was never able to reproduce it on Mac or Windows. So without submitting patches to watchdog (assuming anyone could figure out what the problem is) there isn't anything to be done on that side.
@pkscout mentions here he might update the add-on so I will ping him in case this thread is of any use to him, but there is no response needed.
