Uhm, so in the wiki it says:

Thumbnailing Individual Items in a Single Directory

Name the icon file the same as the file you would like it to display for with a .tbn extension ie: Transformers.avi, Transformers.tbn. This is particularly useful for using custom thumbnails for movies.

Example:

Movies\path\moviename.avi
Movies\path\moviename.tbn

Well, that works. But what if I want to thumbnail a DVD copy on my disk without an avi file in it?

Name it folder.jpg and leave it where it is, or alternatively, rename it to video_ts.tbn and put it inside the VIDEO_TS folder.
